Job Duties and Responsibilities
Your Role as a Home Health Registered Nurse (RN) Clinical Manager:
- Lead interdisciplinary patient assessments and collaborate with the healthcare team to adjust care plans as needed.
- Ensure services ordered by physicians are delivered and monitor patient progress toward goals.
- Educate and counsel patients and families on healthcare needs, care goals, and wellness.
- Oversee timely and accurate clinical documentation, including patient progress and goals.
- Manage patient care services, referrals, and ensure seamless communication between the care team.
- Assign and supervise staff, ensuring the highest quality of care delivery.
- Coordinate with physicians to facilitate discharge planning and optimize patient outcomes.
- Participate in Quality Assessment and Performance Improvement (QAPI) initiatives.
- Ensure adherence to best nursing practices and conduct home visits to assess care quality.
Qualifications
Qualifications We’re Looking For:
- Current RN license in South Carolina or compact multi-state license.
- RN license in good standing.
- Graduate of an accredited nursing program.
- Preferred: 3+ years of home care experience.
- Preferred: 2+ years of management experience.
- Strong background in clinical practice and home care management.
