Comprehensive Home Health is looking for caring and detail oriented Home Health Aides to follow a plan of care to provide personal care services to a patient under the direction of a Registered Nurse, Physical Therapist, Speech Therapist or Occupational Therapist

Job Duties and Responsibilities

  • Demonstrate professional conduct and ethics according to agency policy and procedure.
  • Maintain confidentiality of patient and all Agency operations.
  • Provide patient care in compliance with agency policies and procedures and federal and state regulations.
  • Assist patient with personal hygiene, bathing and grooming.
  • Assist patient with transfer, ambulation and exercise.
  • Assist patient with dressing and toileting.
  • Assist patient with change of colostomy bag.
  • Record intake and output.
  • Assist patient with medications that are self-administered, if Home Health Aide is qualified.
  • Report changes in patient condition in a timely manner to the supervising nurse or Clinical Manager.
  • Provide patient with nutritional support by preparing meals and feeding, if necessary.
  • Assist patient with range of motion and positioning techniques.
  • Take patient’s vital signs including temperature, pulse, and respirations.
  • Maintain a safe and clean environment in patient’s home.
  • Weigh patient.
  • Document patient care accurately in electronic medical record and perform synch daily.
  • Utilize proper body mechanics during provision of patient care.
  • Notify the Clinical Manager or Registered Nurse of inadequate supplies or malfunctioning medical equipment.
  • Complete all annual in-service training and competency assessments as required.
  • Attend staff meetings and case conferences, as requested.
  • Perform other job duties and responsibilities as assigned by the Director of Health
  • Services including participating in performance improvement activities; i.e., record reviews and performance improvement groups.

Qualifications

  • High school diploma or equivalent.
  • Certification as a Home Health Aide.
  • Minimum of one (1) year experience in home health care preferred.
  • Training by an accredited vocational school or hospital; or currently enrolled in an accredited School of Nursing with six (6) month’s clinical experience. Curriculum completed must include nutrition and food management and meet the minimum hours required by state and federal regulations.
  • Background check acceptable to the Agency.
  • Certification of required in-services.
  • Current C.P.R. certification.
  • Valid driver’s license and proof of automobile liability coverage.
  • Successful completion of HHA competency evaluation.
  • Demonstrated organizational skills and detail oriented.
  • Demonstrated time management skills.
  • Sympathetic attitude toward the care of the sick.
  • Ability to read, write, carry out directions.
  • Maturity and ability to deal effectively with the demands of the job.
  • Demonstrated interpersonal skills.
  • Able to communicate in English.
  • Able to work cooperatively as a member of a team.
