Registered Nurse (RN) Case Manager / RN Supervisor – Home Care

BrightStar Care is seeking an experienced RN Case Manager / RN Supervisor to provide clinical oversight, case management, and leadership for our home care teams.

Schedule & Details

  • Monday–Friday | 8:30 AM – 4:30 PM

  • No weekends

  • Local travel required

  • ~75% field / 25% office

  • Rotating on-call schedule

Key Responsibilities

  • Lead SNV and infusion visits

  • Oversee admissions, reassessments, supervisory visits, and case management

  • Clinical lead for CCBW program (Mayo partnership)

  • QA review of non-skilled documentation and caregiver notes

  • Manage infusion intake, scheduling, orders, and POCs

  • Support PDN admissions, reassessments, and occasional shift coverage

  • Assist with chart audits, training support, and DON administrative tasks

Qualifications

What You Need

  • Active Florida LPN license
  • 1+ year nursing experience
