Occupational Therapist (OT) – Home Health

Position Summary

The Occupational Therapist (OT) evaluates patients’ functional activity demands and performance abilities to identify and address deficits. The OT’s goal is to improve quality of life and support the maintenance or restoration of patient independence. Services are provided in accordance with physician orders, the plan of care, and agency standards. This position reports to the Director of Nursing or RN Supervisor.

Key Responsibilities

  • Provide occupational therapy services in accordance with physician orders and the established plan of care

  • Evaluate patients’ functional, rehabilitative, and safety needs

  • Develop and implement individualized plans of care consistent with assessment findings

  • Participate in interdisciplinary treatment planning and patient case conferences

  • Identify, develop, or adapt daily activities to enhance independence and quality of life

  • Assess patient environments with a focus on fall risk, safety, and intervention needs

  • Develop, recommend, or fabricate adaptive equipment to improve function and independence

  • Educate patients, families, caregivers, and staff on treatment goals and use of adaptive equipment

  • Monitor patient progress and report changes in condition to the Director of Nursing and physician

  • Coordinate care with other healthcare providers and care settings as appropriate

  • Prepare and submit accurate clinical documentation and progress summaries at least weekly

  • Participate in discharge planning and identify needs for additional services

  • Provide in-service education to agency personnel as needed

  • Supervise Occupational Therapy Assistants (OTAs) in accordance with policy and state regulations

  • Ensure compliance with all local, state, and federal laws and regulations

Education & Licensure Requirements

  • Graduate of an Occupational Therapy program accredited by ACOTE or predecessor organizations

  • Completion of required supervised fieldwork experience

  • Successful completion of a nationally recognized Occupational Therapy examination

  • Current Occupational Therapist license in the state of practice

  • Compliance with all continuing education requirements

Skills & Qualifications

  • Minimum 2 years of experience as an Occupational Therapist preferred

  • Strong knowledge of occupational therapy principles, evidence-based practice, and AOTA standards

  • Excellent interpersonal, communication, and customer service skills

  • Ability to work collaboratively with patients, families, caregivers, and interdisciplinary teams

  • Commitment to patient dignity, respect, and confidentiality (HIPAA compliant)

  • Adherence to the Occupational Therapy Code of Ethics
