Job Duties and Responsibilities

Patient Care Coordinator job responsibilities include: 

  • Responsible for scheduling patient visits, including confirmation and coordination of daily schedules
  • Support patients via technical support and ensure successful sessions with clinician 
  • Assist clinicians by recording all clinical observations, information provided to the patient, and patient education in EHR software
  • Ensure clear communication of information between clinicians and patient, as well as inter-departmental communication
  • Coordinate clinician-initiated follow-up services with the patient
  • Interact with patients, family members and facility staff in a professional and empathetic manner
  • Strict maintenance of patient confidentiality
  • Complete required training and continuing education courses
  • After completing provided training, assist the Licensed Clinical Psychologist with the administration of cognitive and emotional test instruments


Patient Care Coordinator required skills & qualifications include:

  • Education as a Medical Assistant (MA)
  • Familiarity with behavioral health processes and medical terminology
  • Ability to communicate with patients, families, facility staff and colleagues in a professional and effective manner
  • Computer proficiency including MS Suite fluency and familiarity with Electronic Health Records
  • Ability and willingness to travel to skilled nursing 
  • Valid driver’s license
