Job Overview
We are seeking a dedicated, compassionate, and detail-oriented Licensed Practical Nurse (LPN) to provide high-quality one-on-one care for a female patient in her home. This is a complex clinical case that requires a nurse who thrives in a “busy” clinical environment and enjoys providing personalized attention.Available Shifts
We are looking to fill both Full-Time and Part-Time positions:
- Full-Time: 5 days a week (8-hour shifts).
- Options: 0700–1500 or 1500–2300.
- Part-Time: 2 or 3 days a week (8-hour shifts).
- Options: 0700–1500 or 1500–2300.
- Schedule Note: All positions include rotating weekends.

Job Duties and Responsibilities
Clinical Responsibilities
The patient requires a high level of clinical monitoring and intervention:
- Respiratory Care: Management of a Trach (Trach mask during the day, preset Vent at night). Frequent suctioning (2–3 times per shift) and assistance with a speaking valve.
- Nutrition: Continuous feeding via GJ-tube. Must perform G-tube flushes every hour.
- Wound Care: Management and monitoring of a Stage IV pressure injury on the coccyx.
- Mobility: Mostly bedbound; requires assistance transferring to a wheelchair for a few hours daily.
- Therapy Coordination: Assisting with PT, OT, and Speech Therapy sessions conducted in the home.
Patient & Environment
- Psychosocial Support: The patient experiences anxiety but chooses not to use pharmacological interventions. We are looking for a nurse with strong redirection and emotional support skills.
- Home Dynamic: There is one dog in the home. The patient’s husband has early-stage dementia; the ideal candidate can navigate complex family dynamics with professionalism and empathy.
Qualifications
- Active LPN License.
- Experience with Trach/Vent and G-tube management (required).
- Experience with Stage IV wound care.
- Ability to remain calm and focused in a high-activity home setting.
- Reliability and punctuality are a must.
