Come join our team as we partner with a world-renowned hospital on an exciting and innovative Hospital at Home program. We’re redefining patient care by bringing hospital-level services directly into the home—bridging the gap between acute inpatient care and home health.
We are currently seeking a compassionate and skilled Registered Nurse (RN) with experience in Med-Surg, PCU, Telemetry, ER, or ICU to deliver high-acuity care in the home setting. This is a unique opportunity to be at the forefront of a groundbreaking care model that prioritizes patient comfort, continuity of care, and better outcomes. Shift types are 6:00 – 14:00, 8:00 – 20:00, 12:00 – 24:00 and 15:00 – 23:00.
Job Duties and Responsibilities

•Deliver compassionate, high-quality nursing care in patients’ homes
•Perform comprehensive assessments
•Administer medications, treatments, and procedures
•Monitor patient conditions closely, recognizing early signs of complications and taking proactive steps to prevent deterioration
•Collaborate with physicians, therapists, and the broader care team to coordinate care and optimize outcomes
•Educate patients and their families on disease management, medication use, and self-care
•Use telemedicine tools and remote monitoring technology
Qualifications
•Active and unrestricted Registered Nurse (RN) license in the state of Arizona
•BSN or ADN with relevant experience
•At least 1 year of clinical experience in a Med-Surg, PCU, Telemetry, ER, or ICU setting
•Current Basic Life Support (BLS) certification
